Primary Health Care (PHC) keeps care within reach for 40,000+ patients across Central Iowa each year. Many of the people we serve face real barriers—cost, transportation, language, housing. Your gift removes those barriers so people can get medical, dental, and behavioral health care when they need it.
Beyond the clinic, your support powers two essential safety-net programs:
- Homeless Support Services—the entry point for people without housing in Polk County; connecting them to street outreach, case management, and care.
- The Project of PHC—confidential, low- or no-cost HIV service from testing to treatment and long-term support.

Thanks to our patients, partners, donors and community, PHC has grown into the largest community health center in Iowa, while remaining committed to the highest quality care. In 2024, PHC provided:
Access to Care
- 40,335 patients received care across 10 clinics and service locations.
- Nearly 15,000 under- or uninsured patients received care.
- 139,613 total medical, dental, and behavioral health visits.
Homeless Support Services
- Served 5,000+ individuals experiencing homelessness.
- Completed centralized intakes for 2,500 individuals and families.
The Project of PHC (HIV Services)
- 1,500+ HIV tests for early detection and linkage to care.
- 100% of patients linked to HIV care within 1 month of testing.
- 96% of patients achieved viral suppression.
