Primary Health Care provides a range of services that work together to support patient health, including:

  • Primary medical care for adults and children
  • Dental services
  • Behavioral health care, including therapy
  • Pharmacy services
  • HIV prevention and care services
  • Supportive services that help patients overcome barriers to care

This integrated approach allows patients to access multiple services within one system, with care teams working together to support their health over time.

Focused on Access and Continuity of Care

Primary Health Care continues to focus on making care accessible and consistent for the communities it serves.
PHC accepts many insurance plans, including Medicaid and Medicare, and offers a sliding fee scale for patients who qualify. Patients can access care through in-person visits or telehealth, depending on their needs.

Leadership Perspective

For 45 years, our mission has remained the same: providing high-quality healthcare and supportive services to everyone who needs them. As we look ahead, we remain committed to improving access, strengthening our services, and helping our communities thrive.
