PHC’s Medical Home model of care is a multidisciplinary team of professionals who coordinate care that is unique to each patient.

  • PHC offers flexible scheduling (evenings & Saturdays available at some, but not all clinics)
  • Prompt response to patient requests for medication refills or health related questions

  • Patient access to complete documentation withing one business day
Coordinated Care
  • Ensure the needs of patient are met & supports their health goals in both timing and sequence
  • Support our patients with referrals and recommendations
Quality, Safety & Process Improvement
  • PHC staff are provided safety training upon hire and throughout their tenure at the organization
  • PHC staff utilize safety policies, procedures and follow guidelines to protect our patients, events that occur are tracked
  • Patients receive a survey following their visit to provide feedback, or may contact clinics or administrative to share feedback
  • PHC utilizes the data to inform where improvement is necessary or success is flourishing
Comprehensive Care
  • Provide or facilitate needed care
  • Define and explain roles and responsibilities of primary care medical team, and other specialties they may be referred to
  • Provide disease and chronic care management (diabetes, hypertension, COPD, etc.)
  • Learn about patients social needs (discovering where the patient may need additional resources)
  • Assist our patients with applications or paperwork (to additional resources, applying for eligible benefits etc.)
