Help manage care for your child or loved one
MyChart Proxy Access allows a parent, legal guardian, or approved caregiver to access another patient’s MyChart account.
This can help you stay connected to important health information, send messages, and help manage care.
Who can request Proxy Access?
Children ages 0-11
- Parents or legal guardians may have full access to the child’s MyChart account.
- Legal guardians must provide documentation.
Children ages 12-17
- Parents or guardians may have limited access. This may include the ability to:
- Send and receive messages
- View allergies
- Update communication preferences
- Legal guardians must provide documentation.
Adults who need caregiver support
- Proxy Access may be available for caregivers of adult patients who are unable to make their own health care decisions.
- If you are a healthcare power of attorney or court-appointed legal guardian, PHC must have your legal documents on file before access can be granted.
How to request Proxy Access
- Log in to your MyChart account.
- Open the menu.
- Select Sharing.
- Choose the proxy access option that fits your situation.
- Complete and submit your request.
Legal documents
Legal documents may be required if you are requesting access as a:
- Legal guardian
- Healthcare power of attorney
- Court-appointed guardian
