At Primary Health Care (PHC), we’re proud to celebrate the 10th anniversary of our Centralized Intake program—a vital resource that connects individuals and families experiencing homelessness in Polk County to critical services and support.

Through a streamlined process, our team ensures those in need have access to shelter, housing assistance, and other essential services. By working closely with local agencies, including emergency shelters and housing programs, Centralized Intake helps individuals navigate what can often be a complex and overwhelming system.

Over the past decade, the program has assisted thousands of people in finding stability and hope. By offering one-on-one support, we aim to break down barriers, foster empowerment, and help individuals take steps toward a brighter future.
