At Primary Health Care, we know that staying healthy is about more than doctor visits. Things like food, transportation, housing, and daily support all play a role in your health.
That’s why we work with community partners like the Community Care Hub.
This program has been available for some time and continues to support patients by connecting them to helpful services outside the clinic.
What is the Community Care Hub?
The Community Care Hub helps connect patients to programs and services in the community that support health and daily life.
It works alongside your medical care to make sure you have access to the support you may need to manage ongoing health conditions.
How does it work?
Here’s what to expect:
- Your medical provider can send a referral to the Community Care Hub
- A Navigator will contact you by phone
- They will talk with you about your needs and goals
- The Hub connects you to community programs and resources
- The Hub helps coordinate services and follow up as needed
What kind of help is available?
- Depending on your needs, the Community Care Hub may help connect you to:
- Food and nutrition support
- Transportation resources
- Housing assistance
- Health and wellness programs
- Other community services
